As always, start with history. Since half of all melanomas are initially discovered by the patient,
inquire about new moles or preexisting ones that have recently changed size, color, shape, or sensation. Note that melanomas often develop in melanocytic nevi, but half arise de novo. Tenderness, pain, itching, and bleeding suggest instead more invasive lesions. Ask about personal or family history of melanoma,
tendency to burn, and prior sunburns. On physical exam, rely on (1) the ABCD(E) checklist, and (2) the
revised Glasgow 7-point checklist.
