SLEEP APNEA, OBSTRUCTIVE
1. If obese, prescribe a reducing diet.
2. If nasal obstruction is suspected, treat with fluticasone (Flonase) nasal spray once or twice daily or refer to otolaryngologist for removal of polyps or submucous resection.
3. If collapse of soft tissue in the pharynx is suspected Continuous Positive Airway Pressure (CPAP) may be successful. This can be confirmed by overnight oximetry recordings.
4. If retropositioning of mandible during sleep is suspected, have dentist fit patient with dental appliance to advance mandible and tongue.
5. Consult otolaryngologist for uvulopalatopharyngoplasty, tracheostomy, and other surgical solutions.
