Pathology definition - Patent Ductus Arteriosus
Patent ductus arteriosus
Patent ductus arteriosus occurs as a result of failure of closure of the fetal ductus arteriosus which usually close within 24 hours after birth. This leads to persistent communication between the aorta and pulmonary arteries. Patent ductus arteriosus may lead to left to right shunting due to lower pulmonary vascular resistance.The common complication of patent ductus arteriosus are right ventricular hypertrophy and right heart failure.
Patient ductus arteriosus may occur as a result of congenital rubella. Patient who suffer from patent ductus arteriosus commonly also present with machinery continuous like murmur at the second left intercostal space at the mid clavicular line besides collapsing pulses. The common complication of patent ductus arteriosus are Eisenmenger syndrome and endocarditis of the ductus. Eisenmenger syndrome occur due to reversal of the left to right shunt due to the development of pulmonary hypertension. Eisenmenger syndrome occur due to uncorrected ventricular septal defect and atrial septal defect and commonly present with clubbing, central cyanosis , pulmonary hypertension and decreasing intensity of the murmur.
Indomethacin is useful in closing the patent ductus arteriosus.
