Testicular germ cell tumor
Testicular germ cell tumors may consists of different form such as teratoma, embryonal carcinoma, yolk sac tumor, seminoma and choriocarcinoma.
Teratoma may consists of multiple embryonic layers. Patient may present with gynecomastia and painful lump in the testes. Teratomas are malignant tumor. There will be elevation of hCG and AFP.
Embryonal carcinoma may present with painful mass on the testes. Embryonic carcinoma is a tubular cell / glandular cell patterns tumor with papillary convolution. There will be elevation of hCG.
Yolk sac tumor is also known as endodermal sinus tumor. endodermal sinus tumor may present with core of mesodermis with layers which appear like primitive glomeruli. Endodermal sinus tumor mostly affect children less than 3 years old who present with mass on the testes. There will be elevation of AFP.
Seminoma is similar to ovarian dysgerminoma. Seminoma may present as painless enlargement of the testes. Seminoma may present as sheet of uniform cells, large nucleus and clear cytoplasm. There will be elevation of hCG.
Choriocarcinoma may present as cords of cells with clear cytoplasm. Choriocarcinoma is the most malignant testicular tumor. Choriocarcinoma may consists of cells similar to cytotrophoblast and syncytiotrophoblast. Patient may present with supraclavicular lymphadenopathy, gynecomastia and painless nodular swelling of the testicles. There will be elevation of hCG.
Testicular germ cell tumor mostly affect men between 15 - 40 years old. Patient typically present with testicular dysgenesis or cryptorchidism. The treatment may focus on radical inguinal orchectomy, chemotherapy and radiotherapy.
