Symptom Finder - Treatment of Vaginitis
VAGINITIS
1. Do 10% KOH prep and saline prep of vaginal secretions.
2. If KOH prep is positive for pseudohyphae, treat with fluconazole (Diflucan) 150 mg as a single dose
3. If saline prep shows motile Trichomonas, prescribe metronidazole (Flagyl) four 500 mg tablets as a single dose or 500 mg tid for 7 days.
4. If saline prep shows clue cells or nothing definitive and KOH prep is also negative, treat with metronidazole (Flagyl) 500 mg tid × 7 days or clindamycin 150 mg qid for 7 days. May need to add metronidazole (Flagyl) 500 mg bid to prevent pseudomembranous colitis if clindamycin is prescribed.
