Surgery Definition - What is perilunate dislocation?
Perilunate dislocation may present as the less severe form than lunate dislocation with a very unstable injury however less risk of injury to the median.
Patient may present with fall on the outstretch hand. The common symptoms and signs may include deformity of the wrist with swelling and ecchymoses. There will be paraesthesia in the distribution of the median nerve only if the nerve became stretched due to the dislocation.
Radiological investigation may reveal dislocation of the carpus with lunate in normal position. On lateral view despite perilunate dislocation, the cup remain upright but the 'apple' is out of the cup.
The treatment of perilunate dislocation may focus on the reduction under sedation with below elbow slab and k wires may be needed with or without open reduction internal fixation. Plaster is considered for 6 weeks.
