Surgery Definition – How to perform scalp and occipital examination in secondary survey?
The history should focus on mechanism of injury such as history of penetrating trauma or blunt trauma,the status of immunization and time has elapsed after injury. Then it should focus with identification of the visible injury. Observe for any signs of laceration. Clean and explore any dirt in the wound, foreign bodies. Assess the length and depth of the wound. Any boggy swelling is suggestive of fracture.
Any evidence of assault by weapon, focal neurological deficit, high velocity injury, bruising and marked extensive laceration as well as drop in Glasgow Coma Scale (GCS) may require a skull x ray examination.
Any case of open fracture should be treated with IV antibiotic and constant observation. Neurosurgical consultation is needed. Fracture of the bone under the wound indicates direct contact between body surface and the cranial vault.
Any wound can be closed by pressure dressing and plastic surgery may be considered in scalp avulsion. Any wound need to be closed with staples for speed or non absorbable sutures.
