Ovarian tumor ( Sex Cord Stromal Origin)
Ovarian tumor of sex cord stromal origin may include ovarian fibroma thecoma, granulosa cell tumor and Sertoli - Leydig cell tumor.
The common causes of ovarian tumor of sex cord stromal origin may include family history of ovarian carcinoma, high expression of the HER2/ neu oncogene, nulliparity and mutation of the BRCA 1 and BRCA 2.
Sertoli Leydig cell tumor may composed of Sertoli /Leydig cells which interspersed with the stroma. Sertoli Leydig cell tumor may secretes androgen and may lead to virilism.
Granulosa cell tumor may present with small cuboidal granulosa cells which are arranged in anastomotic cords. There will be the present of small follicle which may filled with secretions of eosinophil or also known as Call - Exner bodies. Granulosa cell tumor may leads to elevation of estrogen level. Granulosa cell tumor secretes estrogen which laed to hyperplasia and carcinoma of the endometrium.Patient maybe presented with endometrial carcinoma, cystic disease of the breast and endometrial hyperplasia which lead to vaginal bleeding.
Ovarian fibroma thecoma is a tumor which may present with Meig syndrome. Meig syndrome may include ovarian tumor, hydrothorax and ascites. Ovarian fibroma thecoma may consists of tumor with round lipid cell with differentiated fibroblast. Ovarian fibroma thecoma may secrete estrogen which lead to elevation in estrogen level.
Generally the treatment may focus on surgically removal of the tumor, oophorectomy, hysterectomy and chemotherapy.
