URINARY INCONTINENCE
1. Rule out urinary tract infection with urine cultures and colony count.
2. Rule out overflow incontinence with prostate examination, ultrasound, or catheterization for residue urine.
3. Look for neurologic signs of pyramidal tract or lower motor neuron disease and if found, refer patient to urologist for cystometric studies.
4. A thorough pelvic examination should be done in females to rule out a cystocele or other pathology.
5. Exercise to build up pelvic muscles (Kegel).
6. Oxybutynin (Ditropan): 5 mg tid–bid.
7. Alternatively, prescribe tolterodine (Detrol) 1–2 mg bid or tolterodine LA (Detrol LA) 2–4 mg daily.
8. Some patient may benefit from dicyclomine (Bentyl) 10 mg qid but need to prescribe
stool softener as well.
