Chronic obstructive pulmonary disease ( COPD)
The common causes of chronic obstructive pulmonary disease are smoking and alpha 1 anti trypsin deficiency.
Chronic obstructive pulmonary disease patient may complain of shortness of breath. Generally patient may present with pursed lip breathing. Hand examination may reveal peripheral cyanosis, bounding pulse ( due to carbon dioxide retention) and nicotine staining. Patient may present with elevated jugular venous pressure, decreased in the cricoid - sternal notch distance and tracheal tug. Chest will be hyper expanded. Crhonic obstructive pulmonary disease is associated with hype resonant on percussion. Expiratory wheeze is common with bilateral decreased in breath sound.
The common complications of chronic obstructive pulmonary disease are pneumonia, pneumothorax, respiratory failure and cor pulmonale.
Patient is advised to stop smoking. beta agonist or anti cholinergic ( bronchodilator) are considered. Other treatments include steroid and long term oxygen therapy.
