Surgery Definition – What is gallstone ileus?
Gallstone ileus is associated with small bowel obstruction by large gallstone in the intraluminal. Gallstone ileus is common in female. Gallstone ileus occurs with large gallstone in the gallbladder which rest against the small bowel. The gallstone will erodes through the wall of both and lie in the lumen of the small bowel and create cholecystoenteric fistula. The gallstone may also descend down the small bowel and causes ileocecal valve obstruction. Gallstone ileus may also occur post endoscopic sphincterotomy.
Gallbladder cancer is also one of the cause of cholecystoenteric fistula.
The classical x ray finding of gallstone ileus may include dilated loop of small bowel, radio opaque gallstone which is distal to the obstruction at the ileocecal junction in the right iliac fossa or the present of air in the biliary tree.
Laparotomy with enterotomy as well as removal of the stone with or without cholecystectomy or cholecystoenteric fistula repair.
