Aortic Stenosis
The common causes of aortic stenosis include , congenital bicuspid aortic valve, rheumatic heart disease present with commissural fusion and senile /degenerative calcification of aortic valve with calcific deposition on the aortic cusps.
Aortic stenosis may present with pulsus parvus et tardus. Pulsus parvus et tardus is a condition where the peripheral pulses ( radial pulses ) are weaker and delayed in nature in comparison with heart sound. Patient may also present with carotid bruit, harsh systolic ejection murmur ( crescendo - decrescendo ) follow by ejection click that radiates from the apex to the neck. There will be an additional heart sound known as S4.
Aortic stenosis may present with complication such as left ventricular hypertrophy, congestive heart failure, syncope and angina. Aortic valve replacement is the treatment of choice in patient with aortic stenosis. Patient also need endocarditis prophylaxis.
