The effectiveness of inotropes and vasopressor are determined by the following steps. It involves the assessment of the neurological status based on the Glasgow Coma Scale with the aim to ensure adequate cerebral perfusion. The temperature and the color of the skin is noted. As well as the capillary refill and peripheral perfusion. The pulse rate and character of the pulse is also noted. Blood pressure and urine output is monitored.
Arterial line is inserted to measure and analyses the beat to beat blood pressure and waveform. Central venous pressure assessment is considered after fluid challenges. Pulmonary capillary wedge pressure is measured using the pulmonary catheter. The contour of the pulse is analyzed with measurement and calculation of the stroke volume, systemic vascular resistance and cardiac output. Esophageal Doppler with application of cross analysis of the flow through aorta is useful as an estimation of the stroke volume and cardiac output.
