Dermatomyositis
Dermatomyositis may present with gottron papules on the elbows, knuckles and knees. There will be a heliotrope rash reddish purple on lids.
The other signs may include weakness of the proximal pelvic and shoulder girdles. Voice may change due to the effect on the pharyngeal and laryngeal muscle.
There will also be swelling, stiffness and tenderness with flexion contractures and muscular atophy.
Intestinal vasculitis in the forms of ulceration and perforation may also present.
The common lab investigation may include muscle biopsy, electromyography, creatine kinase, adolase and LDH levels meaurement.
The treatments of dermatomyositis consist of methotrexate,cyclosporine, corticosteroids, pulse methylprednisolone and cyclophosphamide.
The differential diagnosis of dermatomyositis are systemic lupus erythematosus and juvenile rheumatoid arthritis.
