SYPHILIS
1. Primary: Benzathine penicillin G (Bicillin LA) 2.4 million units IM.
2. For patients who are allergic to penicillin, prescribe doxycycline (Vibramycin) 100 mg bid × 14 days. Ceftriaxone (Rocephin) 1g IM daily for 14 days is also used.
3. Early Latent Syphilis: Benzathine penicillin G 2.4 million units IM one dose.
4. Late latent syphilis and tertiary syphilis: Benzathine penicillin G 2.4 million units IM weekly × 3 doses.
5. Neurosyphilis: Aqueous penicillin G 18–24 million units daily by IV infusion for 10–14 days. Alternatively may treat patients allergic to penicillin with ceftriaxone (Rocephin) 2 g daily IV or IM for 10–14 days.
6. Watch for Jarisch–Herxheimer reaction.
