Abnormalities of the placental attachment.
There are three different forms of abnormalities of the placental attachment. These include placental abruption, placenta accreta and placenta previa.
Placental abruption may present during the third trimester with painful bleeding in the uterus. Placental abruption may result in fatality. Placental abruption may associated with disseminated intravascular coagulation. Placental abruption occur due to premature separation of the placenta. The treatment may focus on controlling the maternal bleeding and considering delivery of the fetal immediately.
Placenta accreta may occur due to inflammation of the endometrium and associated with previous C section scars. Patient may present with massive hemorrhage after delivery. Placenta accreta may require hysterectomy to control the bleeding. In this case, placenta may attach directly to the myometrium due to defective in the decidual layer.
Placenta previa may also occur due to previous scar from the C section. Patient with placenta previa may present with premature labor and painless bleeding. Placenta previa occur due to placenta attachment to the cervix or lower segment of the uterus which later obstruct the cervical os. Placenta previa and placenta accreta may coexist together.
