Pilonidal abscess is treated with incision and drainage. An incision such as elliptical or cruciform is made at the site of pointing (or site of maximal fluctuance which represent point of lack of resistance for discharge of the purulent. The pus will be evacuated. All the pocket of pus or loculations will be broken down with curette or digitally. Betadine or saline solution is considered to irrigate the site. The cavity is packed with ribbon gauze or kaltostat which keep the cavity open and allow granulation to occur from the bottom up. There is a high risk of recurrence and follow up is required.
