In this condition, there is a drop in blood pressure of 20 mm or more on standing. The causes are similar to those discussed under “hypotension” plus disorders of the autonomic nervous system such as diabetic neuropathy and Shy–Dragger syndrome.
Approach to the Diagnosis
A tilt table test is the best way to diagnose this disorder. Blood volume studies, echocardiography, a glucose tolerance test, serum ACTH, and cortisol may need to be ordered as well as a neurology consult.
