Surgery Definition – How to deal with splenic trauma?
Splenic trauma or trauma to the spleen is one of the common causes of intra abdominal bleeding. Splenic trauma may present with laceration, injury to the vessel and hematoma. Splenic trauma can be graded based on its severity. The aim of the treatment is to save the spleen to prevent any further complication such as sepsis after splenectomy. The treatment depends on the hemodynamic stability of the patient and the grade of severity. Splenic trauma can be treated surgically or conservatively. Surgical treatment may include repair of the vascular or splenic injury and consider total splenectomy. Conservative treatment is considered in cases where hemodynamically stability can be achieved. Total splenectomy is considered if more than 2 units of blood transfusion is needed. Following splenectomy pneumococcal and Hib vaccination with prophylactic antibiotic in the form of penicillin are considered.
