INSECT BITES AND STINGS
1. Treat local lesion with triamcinolone 0.1% (Aristocort) cream or ointment: Apply tid to affected area.
2. Treat anaphylaxis and other systemic complications with:
a. Epinephrine: 0.2–0.5 cc of 1:1,000 solution SC or IM or
3–5 cc of 1:10,000 solution IV.
b. Diphenhydramine (Benadryl): 25–50 mg IV q2–3hrs PRN.
c. Methylprednisolone succinate (Solu-Medrol): 125 mg IV q6hrs.
3. Antivenom for scorpion and black widow spider bites.
4. Follow patients with tick bites for Lyme disease, Rocky Mountain spotted fever, etc.
5. Treat catecholamine release in scorpion bites with nitroprusside or β-blockers.
6. Observe bites for secondary bacterial infections and treat with antibiotics.
