Surgery Definition – What is diagnostic peritoneal lavage?
Diagnostic peritoneal lavage is useful in indicating the present of intra abdominal hemorrhage and perforation of the intestine. Diagnostic peritoneal lavage is useful in cases of unexplained hypotension or hypovolemia or when the patient is undergoing prolong extra abdominal procedure.
Diagnostic peritoneal lavage is performed by inserting catheter into an incision in the lower and midline of abdominal wall and aspiration is later performed. Aspiration is performed every 10 minutes. Bladder need to be emptied first and notes for any feculent or blood in the aspiration. 10 ml/ kg of warm saline is irrigated through the catheter.
Positive diagnostic peritoneal lavage is identified with more than 5 ml of frank blood on initial aspiration, more than 100 000 red blood cells per mm3 in the lavage fluid and more than 500 white blood cells per mm3 in lavage fluid.
The contraindication of diagnostic peritoneal lavage may include, previous surgery, coagulopathy, advanced stage of pregnancy, ascites and gross obesity.
