Wegener Granulomatosis
Wegener granulomatosis may present with signs and symptoms such as chronic sinusitis, hemoptysis, nasal septum perforation, dyspnea, otitis media, rash, myalgia and hematuria. Wegener granulomatosis is common in middle age men.
Wegener granulomatosis may present with fibroplastic proliferation surrounding the inflammatory infiltrates of the granulomatous vasculitis. Besides that vascular granulomatous lesions may be appear on the lung, kidney and upper respiratory tracts. There will be a progression into glomerulonephritis.Histopathological analysis may reveal necrotizing granulomas. The necrotizing granulomas may present in the wall of the vascular with necrotic center which is surrounded by giant cells, macrophages and lymphocytes.
Wegener granulomatosis is treated with cyclophosphamide, prednisolone and methotrexate. Wegener granulomatosis commonly occur due to immunological reaction such as hypersensitivity reaction. Investigation may reveal the present of red cell cast in the urine. There will be nodular detected on the chest x ray and C- ANCA is detected on blood /antibodies analysis.
