Surgery Definition – How to clinically assess shock?
Shock can be accessed by considering look, listen and feel. Look may involve looking for any signs of peripheral cyanosis or central cyanosis. A cold and clammy patient is associated with peripheral vasoconstriction due to hemorrhage as a result of sympathetic response. Look for nay signs of visible trauma and hemorrhage, count respiratory rates which should be 14 -20 in normal adult, inspect the signs of distention of the jugular vein due to cardiogenic shock Consider identifying jugular venous pressure and look for any evidence of cerebral hypoxia such as confusion, aggression, drowsiness and coma.
Listen for any muffled heart sounds and feel the pulse( radial pulse, carotid pulse or femoral pulse) and notice the rate which normally suppose to be 60 – 100, rhythm either regular or irregular and volume of the pulse such as bounding, absent or thready. Absent of pulse indicates cardiac arrest and BLS ( basic life support ) is considered. Need to press the sternum for 5 seconds and observe the time taken for the blanch to return to normal color ( normally 2 second).
