Surgery Definition - What is avascular necrosis?
Avascular necrosis may present as tenderness in the anatomical snuffbox and pain on telescoping the thumb. The common test performed is known as Chen's scaphoid compression test. In this test the force applied axially along the thumb. Pain will present due to the metacarpal of thumb hitting the injured scaphoid bone with which it articulates.
Avascular necrosis is also common in the femoral head, humeral head and talus in the foot. Avascular necrosis may lead to osteoarthritis at the carpo radial joint.
The radiological investigation of scaphoid views is needed. On follow up, in 2 weeks time radiological x ray is required to detect any increase in severity of the fracture due to osteopenia around the area of fracture.
The treatment of avascular necrosis may include symptomatic treatment if the fracture appears to be not clear on the x ray or considered scaphoid plaster for 6 weeks with hand to be held in the cup holding position in case of scaphoid fracture is confirmed.
