ANY OBVIOUS DEFORMITY OR OUTSTANDING FEATURES
Points to be noted.
Be careful to note these points:
Halitosis (pyorrhoea alveolaris, bronchiectasis, hepatic encephalopathy)
Shaking hands (static tremor in parkinsonism)
Continuous crying (i.e., from pain anywhere in body, fear, or emotional upset)
Instability on standing (ataxia)
Asymmetry in face while talking or smiling (Bell’s palsy)
Sudden bending of legs while lying (flexor spasm in a paraplegie patient)
Restlessness (akathisia in thyrotoxicosis)
Head nodding (tics, mental retardation, cerebellar disorder, parkinsonism)
Deformity of skull (macrocephaly in hydrocephalus, or microcephaly in cerebral palsy), or severe kyphoscoliosis
Semi-purposive, rapid, jerky movements of upper limb (chorea)
Breathlessness (e.g., pleural effusion, pneumothorax, pericardial effusion, pulmonary
hypertension, huge ascites or hysteria)
Squatting (indicating cyanotic congenital heart diseases)
Wheezing (smaller airways obstruction e.g., bronchial asthma)
Huge swelling of abdomen (ascites)
Stooped posture (parkinsonism, ankylosing spondylitis, osteoporosis of spine with
vertebral collapse).
